Objective To evaluate feasibility, acceptability, and immediate patient-reported outcomes of Dental Confidence , a web-based risk communication tool addressing anticipatory dental anxiety in the context of medication-related osteonecrosis of the jaw (MRONJ) in older adults receiving antiresorptive therapy. Methods In this prospective single-arm explanatory sequential mixed methods study, 51 participants completed assessments before and immediately after application use. Five participants were purposively selected for semi-structured interviews to contextualize contrasting quantitative response patterns. Results All participants completed the intervention and assessments. Mean Visual Analog Scale (VAS) scores were 53.24 (SD 32.62) before application use and 44.80 (SD 33.03) immediately after use, with a mean paired change of −8.43 points (95% CI −13.16 to −3.70; P < 0.001; Cohen's dᶻ = 0.50). Self-reported knowledge and communication confidence scores were unchanged (25.49 [SD 4.12] vs 25.43 [SD 4.56]; P = 0.90). Mean satisfaction was 4.45 (SD 0.81) on a 5-point scale, and 46 of 51 participants (90.2%) rated satisfaction as 4 or 5. Qualitative explanatory categories suggested that reassurance, risk contextualization, visual comfort, and communication support may help contextualize variation in the observed VAS response patterns. Conclusion A web-based risk communication tool was feasible and highly acceptable in a supported outpatient setting and was associated with lower immediate scenario-based VAS anxiety scores in the context of MRONJ risk. Controlled studies with longer follow-up are needed. Innovation This study introduces a bilingual, older adult-oriented, rule-based digital tool and extends patient-facing risk communication to an underaddressed medication-related dental concern, while using mixed methods to contextualize variation in immediate anxiety responses.
