Abstract Background Population ageing poses increasing challenges for health systems, particularly in low- and middle-income countries. In Thailand, rapid demographic transition underscores the need for proactive strategies targeting middle-aged populations. However, practical community-level models for individuals aged 50–59 years remain limited. This study aimed to develop and evaluate a community-based participatory model to enhance multidimensional ageing readiness in a Thai context. Methods This study employed a participatory action research (PAR) design conducted in a community setting in Thailand. The research was implemented in three phases: (1) situational analysis of ageing preparedness among pre-older adults; (2) participatory development of a community-based ageing preparation model; and (3) implementation and evaluation of the model. Stakeholders included local government representatives, healthcare providers, community leaders, and pre-older adults. The model addressed five domains of ageing readiness: health, social participation, economic security, living environment, and technology use. Data were collected through surveys, participatory workshops, and evaluation assessments. Quantitative data were analysed using descriptive statistics, while qualitative data were analysed using thematic analysis. Results The study resulted in the development of a community-integrated ageing preparation model that integrates health promotion, social engagement, financial planning, environmental adaptation, and technology literacy. A total of 330 pre-older adults participated in the study. Post-intervention assessment showed a significant improvement in overall ageing readiness, increasing from a mean score of 3.12 ± 0.54 to 3.58 ± 0.49 (mean difference = 0.46, p < 0.001), with a large effect size (Cohen’s d = 0.86). Improvements were observed across all five domains of readiness. Stakeholder participation facilitated locally appropriate strategies, strengthened collaboration between community organizations and health services, and enhanced coordination in implementing ageing-related initiatives. The model was perceived as feasible for integration into local health and social support systems. Conclusions A community-based participatory approach can effectively support ageing preparedness at the local level by integrating multiple dimensions of wellbeing and engaging key stakeholders in model development and implementation. The proposed model provides a practical framework for strengthening community readiness for population ageing and may inform policy and program development in similar community and health system contexts. Trial registration: Not applicable. This study employed participatory action research design and did not involve a clinical trial or controlled intervention requiring registration.
