Abstract Topic Esophageal Cancer: Other Background Smoking is a major risk factor for esophageal squamous cell carcinoma (ESCC). While long-term smoking increases ESCC risk, the effect of smoking cessation duration among low-dose smokers remains unclear. This study aimed to evaluate the association between smoking duration and cessation period among ESCC patients. Methods A retrospective cross-sectional analysis was conducted among 100 ESCC patients stratified by smoking duration (≤20 years, 20–40 years, ≥40 years) and smoking cessation period (≤5 years, 5–10 years, ≥10 years). A chi-square test of independence was performed to compare distributions across groups. Effect size was calculated using Cramér’s V Results A statistically significant association was observed between smoking duration and cessation period (χ2 = 29.18, df = 4, p < 0.0001; Cramér’s V = 0.38). Patients with long smoking duration (20–40 and ≥40 years) were predominantly clustered in the short cessation group (≤5 years). Notably, low-dose smokers (≤20 years) demonstrated the highest proportion of ESCC cases in the long-term cessation group (≥10 years cessation: 18%), suggesting persistent cancer risk despite prolonged abstinence. Even after ≥10 years of cessation, ESCC cases remained evident among short-duration smokers. Conclusion Smoking duration significantly influences the temporal pattern of ESCC occurrence after cessation. Importantly, low-dose smokers remain at persistent risk of ESCC despite long-term cessation, highlighting the need for continued surveillance strategies regardless of smoking intensity or cessation duration.
