Background: Topical capsaicin, approved by the FDA for treating musculoskeletal pain, is effective for localized postoperative neuropathic pain. However, over 60% of patients discontinue treatment due to burning adverse effects. To enhance tolerability, capsaicin-loaded niosomes have been developed. Nevertheless, the clinical efficacy and safety of these niosomes in patients have not yet been studied. Objective: To evaluate the efficacy and safety of 0.025% capsaicin-loaded niosomes (CLN), administered either as monotherapy or in combination with oral neuropathic pain medications, in patients with postoperative neuropathic pain following cancer surgery. Methods: This prospective, open-label study enrolled patients with clinically confirmed postoperative neuropathic pain. Eligible participants received 0.025% CLN. The primary endpoints were mean pain score, difference in pain score, and percentage change in pain score, assessed using the Neuropathic Pain Symptom Inventory-Thai version (NPSI-T) and visual analog scale (VAS). Secondary endpoints included overall treatment satisfaction and incidence of local adverse events. Data were collected at baseline, week 4, and week 8. Subgroup analyses compared outcomes between the monotherapy and combination therapy groups. Results: Twenty-six patients were enrolled (14 in the CLN monotherapy group and 12 in the combination therapy group). Mean pain scores for all patients decreased significantly over time (p
