Immunodominant B-cell epitopes for precision surveillance of Plasmodium vivax malaria

This study integrates computational immunology and serology to identify P. vivax epitopes for immune-exposure surveillance, moving from 142 proteins to six antigens and 64 synthesised epitopes tested across multiple regions.

Key findings

  • Ten immunodominant epitopes emerged. MSP9_603–609 reached 90.4% sensitivity and AUC 0.980; AMA1_457–466 reached AUC 0.910; MSP9_425–433 reached AUC 0.898; and MSP9_384–389 discriminated asymptomatic infection with AUC 0.756. Cross-regional detection ranged from 2.2% to 22.2%.

Why this matters globally

Highly conserved epitopes could inform serosurveillance tools for exposure mapping and hidden-transmission detection in malaria-elimination programmes, but field-validated assays and thresholds are still required.

Limitations to consider

Sample sizes and thresholds require full-text review. Regional detection varied widely. Serology reflects immune exposure and may not distinguish current from past infection. Specificity against P. falciparum in tested samples does not cover all infections, and no point-of-care prototype or prospective field evaluation is reported.
