Embracing Grief: Understanding Perinatal Guilt, Shame, and Coping Behaviours After Miscarriages

Background: Miscarriages can lead to profound psychological distress, particularly in culturally sensitive settings like Pakistan, where motherhood is highly valued. Women often face stigma, self-blame, and isolation, intensifying feelings of guilt and shame, which profoundly influence their psychological well-being and coping mechanisms. Objectives: To examine the effects of guilt, shame, and coping behaviour among women after miscarriages in Pakistan. The study aims to provide valuable insights into factors associated with perinatal mental health. Methods: The study employed a cross-sectional survey design and a purposive sampling technique. Online surveys were conducted from 139 women (aged 18-55) who had experienced at least one miscarriage. The sample size was calculated using the sample size calculator by Raosoft, Inc. Women with a history of mental illness before miscarriage were excluded from the study. Data were collected through an Urdu questionnaire comprising a demographic information sheet, the 8-item State Shame and Guilt Scale (SSGS-8), and the Brief-COPE inventory. Descriptive and inferential statistics were computed using the statistical software SPSS 27. Results: The study found that guilt and shame are significant positive correlates and predictors of each other among women who have had at least one miscarriage. Guilt negatively affected problem-focused and emotion-focused coping. Problem-focused coping was more common among older women and those who had experienced more miscarriages. Significant differences in coping strategies were found between women who experienced emotional problems after a miscarriage and those who did not. Conclusion: The study highlights the effects of guilt, shame, and coping strategies among women having miscarriages. It…

Tin Dichalcogenides as Bifunctional Catalysts for Electrochemical Water Splitting in an Alkaline–Saline Medium

Electrochemical water splitting stands out as a viable method for generating sustainable hydrogen to address the energy needs of the future. This study evaluates mechanochemically synthesized tin dichalcogenides SnSe 2 and SnS 2 as bifunctional electrocatalysts. SnSe 2 and SnS 2 modified nickel foam (NF) electrodes show overpotential values of 378 and 307 mV for the oxygen evolution reaction and 124 and 103 mV for the hydrogen evolution reaction, respectively, at a current density of 10 mA cm −2 . Further, SnS 2 was tested in a symmetric two-electrode configuration, SnS 2 /NF||SnS 2 /NF, which delivers an overall cell voltage of 1.677 V at 10 mA cm −2 and stability over 35 h at 20 mA cm −2 . The findings highlight the promise of mechanochemically synthesized SnS 2 as an economical and effective bifunctional electrocatalyst for electrochemical water splitting. To explore the relevant performance in saline conditions, alkaline–saline model electrolytes are considered, wherein 0.5, 1.0, and 1.5 M NaCl + 1 M KOH electrolytes were tested, revealing that 1 M NaCl + 1 M KOH yields lower OER and HER overpotential values of 334 and 125 mV, and the SnS 2 /NF||SnS 2 /NF cell remained stable over 35 h.

Factors predicting medication adherence among family caregivers of people with schizophrenia and type 2 diabetes mellitus: A hospital-based cross-sectional study

BACKGROUND: Medication adherence remains a significant challenge among people with schizophrenia, particularly those having comorbidity with type 2 diabetes mellitus (T2DM) due to complex treatment regimens and long-term care demands. Family caregivers play a central role in medication management; however, evidence regarding how caregiver-related factors influence medication adherence is limited. OBJECTIVE: This study examined the factors predicting medication adherence among family caregivers of people with schizophrenia and T2DM. METHOD: A hospital-based cross-sectional study was conducted among 530 family caregivers of people with schizophrenia and T2DM receiving outpatient psychiatric services in seven provinces in Northeastern Thailand. Data were collected using validated questionnaires assessing sociodemographic characteristics, caregiver-related psychosocial factors, and medication adherence. Multivariable linear regression analysis was performed to identify factors predicting medication adherence, with a significance level of 0.05. RESULTS: The final model explained 41.0% of the variance in medication adherence. Higher caregiver mental health literacy (MHL) and access to health information via the internet were positively associated with medication adherence, whereas higher family caregiver income and sense of coherence were negatively associated with medication adherence. CONCLUSIONS: Medication adherence among people with schizophrenia and T2DM was associated with family caregiver-related psychosocial and socioeconomic factors. Family caregiver-centered interventions that enhanced MHL, promoted access to reliable health information, and considered family caregivers' socioeconomic contexts may improve medication adherence and long-term treatment outcomes in…

Proteomic profiling reveals mitochondrial metabolic alterations in dexamethasone-induced neuronal differentiation

ObjectiveAs an in vitro model of catecholaminergic neurons, Cath.a Differentiated cells or CAD cells have been selected because of their direct origin in the mouse CNS and their ability to undergo inducible differentiation. Dexamethasone (DEX), a glucocorticoid receptor agonist, generates postmitotic and neurite-bearing CAD cells. Although the morphological differentiation of CAD cells induced by DEX has been well characterized, a comprehensive understanding of its proteomic profile and underlying pathways remains limited. Neuronal differentiation involves substantial remodeling of mitochondrial metabolic programs. However, the relationship between DEX-induced neuronal differentiation in CAD cells and mitochondrial metabolic state remains incompletely understood.MethodsIn this study, we applied a label-free quantitative SWATH-MS proteomic approach to investigate the protein expression in CAD cells upon differentiation.ResultsThe results of the proteomic analysis of 1,114 proteins associated with various GO terms, including neuronal differentiation and characteristics of brain-derived CAD cells, are shown. The data revealed the upregulation of proteins involved in mitochondria-associated metabolism and oxidative phosphorylation in DEX-differentiated cells compared with those in dividing cells, highlighting the role of mitochondrial metabolic changes during the neuronal differentiation of CAD cells. In addition, we identified proteins that were commonly expressed between the two neuronal differentiating protocols. A shared set of proteins involves electron transport, metabolic pathways, and DNA repair.ConclusionsThis study first highlights the proteomic signature and elucidates the key altered molecular pathways underlying DEX-induced neuronal differentiation in CAD…

International Implementation of ASIARS-Net for AMR Surveillance: Building on JANIS

OBJECTIVES: This study aimed to provide a narrative review of the history, characteristics, and progress of the ASIan Antimicrobial Resistance Surveillance Network (ASIARS-Net) system, an international web-based database system for antimicrobial resistance (AMR) surveillance. METHODS: ASIARS-Net was compared with other surveillance platforms, particularly WHONET, a globally used AMR surveillance software supported by the World Health Organization, in terms of usability, AMR data analysis and outputs, and data management and integration, to clarify their complementary roles. The comparison was conducted based on experiences in countries that have implemented ASIARS-Net, enabling each participating country or hospital group to maintain a confidential database, process surveillance datasets from WHONET or Excel files, and generate national summaries and facility-level feedback reports. RESULTS: In Thailand, WHONET data from 74 hospitals covering 840,000 specimens, along with Excel data from three hospitals-each processing over 20,000 isolates annually-were processed to generate feedback reports tailored to user needs and to enable inter-hospital comparisons. In Vietnam, the use of ASIARS-Net together with WHONET or Excel is independently coordinated by laboratories. In 2024, eight hospitals successfully uploaded data and generated feedback reports, covering more than 140,000 bacterial isolates. Moreover, a nationwide clinical laboratory reported on more than 90,000 isolates collected during 2022-2024. These reports not only showed clear trends but also highlighted higher isolation and resistance rates of specific inpatient AMR pathogens in Vietnam than in Japan. CONCLUSION: The ASIARS-Net system is evolving into a globally relevant platform complementary to WHONET for AMR…

Association Between Bladder Volume Changes and Organ-at-Risk Dose Constraints in a Moderate Hypofractionation for Prostate Cancer

Background: The aim of the study was to evaluate the association between bladder volume and dose-volume constraints of organs at risk—specifically the bladder and rectum—in patients receiving moderately hypofractionated radiotherapy for localized prostate cancer, and to identify factors associated with exceeding organs-at-risk dose constraints. Methods: This retrospective study included patients who received either 70 Gy in 28 fractions or 60 Gy in 20 fractions using volumetric modulated arc therapy with daily cone beam computed tomography. Bladder volume was measured at each session. Multilevel mixed-effects linear regression was used to assess the association between bladder volume and organs-at-risk dose constraints. Zero-inflated Poisson regression was applied to explore factors associated with organs-at-risk dose overdose, defined according to the Radiation Therapy Oncology Group (RTOG) 0415 and Conventional or Hypofractionated High Dose Intensity Modulated Radiotherapy in Prostate Cancer (CHHiP) trial thresholds. Results: A total of 388 radiotherapy sessions were analyzed among 15 patients. Increased bladder volume was significantly associated with reduced radiation exposure to organs at risk. This protective effect was more pronounced for the urinary bladder (risk difference: −6.4% to −7.8% per 100 mL increase) than for the rectum (−1.0% to −2.2%) under the 70 Gy protocol. No significant rectal protection was observed under the 60 Gy protocol. The 60 Gy protocol (risk ratio = 15.4; 95% confidence interval, 2.93–81.02) and high recurrence risk (risk ratio = 2.41; 95% confidence interval, 1.12–5.16) were significant predictors of bladder overdose. The 60 Gy protocol was the only significant predictor of rectal overdose. Conclusions: Larger bladder volumes reduce…

Prevalence of insomnia symptoms and mediation of the association between multimorbidity and insomnia symptoms by loneliness and multisite pain among middle-aged and older adults in Malaysia

The aim of the study is to estimate the prevalence and associated factors of insomnia symptoms among middle-aged and older adults in Malaysia. In all, 5602 cross-sectional nationally representative data from adults (≥40 years) of the 2018 Malaysia Aging and Retirement Survey (MARS) was analyzed. Insomnia symptoms were assessed with three items from the Jenkins Sleep Scale (JSS-4). Unconditional multivariable logistic regression was used to identify relationships between social and health factors and insomnia symptoms. To examine the indirect effects of multimorbidity on insomnia symptoms via loneliness and multisite pain, causal mediation analyses were conducted. The prevalence of insomnia symptoms was 29.0%. In the final adjusted model, urban residence (AOR: 1.25, 95% CI: 1.10–1.43), multisite pain (AOR: 1.18, 95% CI: 1.04–1.35), loneliness (AOR: 1.11, 95% CI: 1.08–1.14), poor self-rated health (AOR: 1.16, 95% CI: 1.11–1.22), incontinence (AOR: 1.91, 95% CI: 1.56–2.33), and multimorbidity (AOR: 1.21, 95% CI: 1.05–1.39) were positively associated with insomnia symptoms, while male sex (AOR: 0.88, 95% CI: 0.77–0.99), Islamic religion (AOR: 0.75, 95% CI: 0.66–0.86), has caring friends (AOR: 0.91, 95% CI: 0.85–0.98), and higher physical activity (AOR: 0.96, 95% CI: 0.94–0.98) were negatively associated with insomnia symptoms. Furthermore, there was a significant indirect effect of loneliness (explaining 29.2%) and multisite pain (explaining 30.3%) on insomnia via multimorbidity. Almost one in three aging adults reported insomnia symptoms. Loneliness and multisite pain were found to mediate the association between multi-morbidity and insomnia problems. Reducing loneliness and multisite pain may help in reducing insomnia symptoms in aging multi-morbid patients.

When dying worms look alike: Convergent pathology may create mechanistic illusion in plant-derived anthelmintic research

ABSTRACT The recent study by Devi et al. provided compelling ultrastructural and histological evidence of severe damage in Ascaridia galli following exposure to Meyna laxiflora seed extract, supporting its anthelmintic potential. However, the interpretation of such morphological alterations warrants further consideration. Similar cuticular and internal lesions have been reported in parasites exposed to chemically unrelated compounds with distinct molecular targets, suggesting that comparable structural phenotypes may represent convergent pathological endpoints rather than mechanistic signatures. This raises the possibility that morphology-based interpretations could inadvertently create a false mechanistic consensus in plant-derived anthelmintic research. Histopathology and scanning electron microscopy are powerful tools for demonstrating efficacy, but they may not reveal the initiating biological events underlying parasite death. Distinguishing convergent pathology from shared pharmacology is therefore essential for the rational development of botanical anthelmintics. Future investigations should focus on mechanism-oriented approaches capable of identifying upstream molecular and physiological disturbances preceding structural collapse. Understanding why dying worms look alike may prove as important as discovering new compounds capable of killing them.

Prospective feasibility study of a telehealth platform for community-based NCD risk assessment by community health leaders in rural northern Thailand

Introduction Limited access to preventive healthcare in rural and remote areas poses a persistent challenge for early identification of non-communicable disease (NCD) risk. We evaluated the feasibility and acceptability of HealthD , a telehealth-supported platform that enables Community Health Leaders (CHLs) to conduct community-based NCD risk assessment and facilitate follow-up in rural northern Thailand. Methods We conducted a prospective, single-arm feasibility and acceptability study from June to November 2023 in Chiang Mai Province, Thailand. A total of 120 adults aged 30-70 years without a prior recorded diagnosis of the target NCDs were enrolled. Trained CHLs used HealthD to perform community-based assessment for diabetes mellitus (DM), cardiovascular disease (CVD), and respiratory symptom/functional limitation screening using guideline-informed algorithms and field-based measures, followed by risk-tailored counseling or teleconsultation at two- and four-month follow-up. Feasibility was assessed through recruitment, retention, CHL competency, workflow delivery, and acceptability among CHLs and participants. Results Of 126 individuals approached, 120 were enrolled (recruitment rate 95.2%), and 119 completed follow-up (retention rate 99.2%). CHL competency improved after training, with mean assessment scores increasing from 8.4 to 9.3 ( p = 0.010). Acceptability was high among CHLs (92%) and participants (97%). The HealthD workflow, including in-person assessment, point-of-care testing, digital data entry, algorithm-based risk-output generation, and teleconsultation follow-up, was implemented in the study setting. NCD-related risk outputs are presented as descriptive, exploratory findings. At baseline, 73% of participants were classified for DM-related risk, 3%…

Cytotoxic Effect of Halimeda macroloba Crude Extract on Cervical Cancer Spheroid Cells via p-mTOR/mTOR, Apoptosis, and Ferroptosis Pathways

Background: Because of the limited efficacy of existing therapeutic strategies and the emergence of treatment resistance, cervical cancer remains a major global health burden. Recently, marine-derived natural compounds have attracted attention as potential sources of novel anticancer agents. We aimed to investigate the cytotoxic effects of crude Halimeda macroloba extract on cervical cancer spheroid cells and to explore its underlying molecular mechanisms. Methods: Three-dimensional spheroid cultures of SiHa cervical cancer cells were treated with varying concentrations of H. macroloba extract, followed by assessment of migration, cell viability, caspase activity, and protein expression profiles using liquid chromatography–tandem mass spectrometry, with selected findings validated by Western blotting. Results: Treatment with H. macroloba extract significantly reduced spheroid migration and decreased cell viability in a dose-dependent manner, accompanied by increased caspase activity. Proteomic analysis demonstrated altered expression of proteins associated with mitochondrial function, apoptosis-related pathways, and oxidative stress-related processes. Mechanistic studies further revealed downregulation of phosphorylated mTOR (p-mTOR) and total mTOR, together with increased expression of pro-apoptotic markers. Conclusion: The H. macroloba crude extract exerts anticancer effects in cervical cancer spheroids, primarily through apoptosis-associated mechanisms and modulation of the mTOR signaling pathway, supporting its potential for further investigation as a marine-derived bioactive compound. Keywords: cervical cancer, natural extracts, proteomic analysis, anticancer agents

Chikungunya virus infection: A comprehensive review of pathogenesis, diagnosis and management

Chikungunya fever (CHIKF), caused by the chikungunya virus (CHIKV), is an acute mosquito-borne viral disease that poses a rising global public health threat. To date, no specific antiviral therapies are available, and precise diagnosis is pivotal for optimal clinical management. This review systematically summarizes the latest evidence on CHIKV pathogenesis, diagnostic strategies, and clinical management based on English literature published between 2004 and 2025. Virus-host interactions dominate disease progression, including viral entry mediated by E1/E2 envelope proteins binding to host Mxra8 receptors and suppressed type I interferon responses via nsP2-mediated inhibition of the JAK-STAT pathway. The E1-A226V mutation greatly enhances viral transmissibility in Aedes albopictus, fueling the global spread of CHIKV. Clinically, persistent chronic arthropathy serves as the major long-term complication in 25%–40% of infected patients. Current diagnostic approaches consist of real-time RT-PCR as the early gold-standard test and serological antibody detection, whereas serological cross-reactivity with other arboviruses and inconsistent detection results frequently cause clinical misdiagnosis, and the confirmatory plaque reduction neutralization test (PRNT) is impractical for routine use. Clinical care for CHIKF remains predominantly supportive, relying on symptomatic treatment and close patient monitoring. Two FDA-approved vaccines have been licensed via accelerated approval based on immunogenicity surrogates, with solid clinical protective efficacy yet to be validated. This review highlights that in-depth elucidation of CHIKV pathogenic mechanisms, refinement of diagnostic accuracy, and development of targeted therapeutics are essential to improve the overall management…

Loneliness, rumination, and adolescent psychological crisis in China: a pilot moderated mediation study

Background Adolescent psychological crisis—encompassing depressive disorder, anxiety disorder, and suicidal ideation—is a major public health challenge. One in five adolescents experiences a clinically significant mental health condition globally. Loneliness is a well-documented, modifiable risk factor for these outcomes. However, the exact cognitive-emotional pathways linking loneliness to acute psychological crisis remain unclear. This is particularly true in China, where rapid social change and intense academic pressure increasingly strain adolescent mental health. We do not yet fully understand how protective resources might buffer these specific pathways. Objectives and methods This pilot study tests a moderated mediation model based on the Evolutionary Theory of Loneliness and the stress-vulnerability framework. We investigate ruminative thinking as a cognitive-emotional mediator between loneliness and psychological crisis. We also test whether perceived social support and psychological resilience moderate distinct stages of this pathway. Based on a pilot sample of N = 312 Chinese adolescents (ages 12–18), we use structural equation modelling (SEM), confirmatory factor analysis (CFA), and Hayes’ PROCESS Macro (Model 14) with 5, 000 bootstrap replications. Two-wave longitudinal data ( n = 187, 8-week interval) were available for the depression outcome only; the anxiety and suicidal-ideation pathways were examined cross-sectionally. Results Loneliness is significantly associated with depressive symptoms (β = 0.43, p < 0.001), anxiety (β = 0.38, p < 0.001), and suicidal ideation (β = 0.27, p < 0.001). Ruminative thinking accounts for 40–46% of the total indirect association across all three outcomes. Social support buffers the transmission from loneliness to…
