Association Between Bladder Volume Changes and Organ-at-Risk Dose Constraints in a Moderate Hypofractionation for Prostate Cancer

Background: The aim of the study was to evaluate the association between bladder volume and dose-volume constraints of organs at risk—specifically the bladder and rectum—in patients receiving moderately hypofractionated radiotherapy for localized prostate cancer, and to identify factors associated with exceeding organs-at-risk dose constraints. Methods: This retrospective study included patients who received either 70 Gy in 28 fractions or 60 Gy in 20 fractions using volumetric modulated arc therapy with daily cone beam computed tomography. Bladder volume was measured at each session. Multilevel mixed-effects linear regression was used to assess the association between bladder volume and organs-at-risk dose constraints. Zero-inflated Poisson regression was applied to explore factors associated with organs-at-risk dose overdose, defined according to the Radiation Therapy Oncology Group (RTOG) 0415 and Conventional or Hypofractionated High Dose Intensity Modulated Radiotherapy in Prostate Cancer (CHHiP) trial thresholds. Results: A total of 388 radiotherapy sessions were analyzed among 15 patients. Increased bladder volume was significantly associated with reduced radiation exposure to organs at risk. This protective effect was more pronounced for the urinary bladder (risk difference: −6.4% to −7.8% per 100 mL increase) than for the rectum (−1.0% to −2.2%) under the 70 Gy protocol. No significant rectal protection was observed under the 60 Gy protocol. The 60 Gy protocol (risk ratio = 15.4; 95% confidence interval, 2.93–81.02) and high recurrence risk (risk ratio = 2.41; 95% confidence interval, 1.12–5.16) were significant predictors of bladder overdose. Prevalence of insomnia symptoms and mediation of the association between multimorbidity and insomnia symptoms by loneliness and multisite pain among middle-aged and older adults in Malaysia

The aim of the study is to estimate the prevalence and associated factors of insomnia symptoms among middle-aged and older adults in Malaysia. In all, 5602 cross-sectional nationally representative data from adults (≥40 years) of the 2018 Malaysia Aging and Retirement Survey (MARS) was analyzed. Insomnia symptoms were assessed with three items from the Jenkins Sleep Scale (JSS-4). Unconditional multivariable logistic regression was used to identify relationships between social and health factors and insomnia symptoms. To examine the indirect effects of multimorbidity on insomnia symptoms via loneliness and multisite pain, causal mediation analyses were conducted. The prevalence of insomnia symptoms was 29.0%. In the final adjusted model, urban residence (AOR: 1.25, 95% CI: 1.10–1.43), multisite pain (AOR: 1.18, 95% CI: 1.04–1.35), loneliness (AOR: 1.11, 95% CI: 1.08–1.14), poor self-rated health (AOR: 1.16, 95% CI: 1.11–1.22), incontinence (AOR: 1.91, 95% CI: 1.56–2.33), and multimorbidity (AOR: 1.21, 95% CI: 1.05–1.39) were positively associated with insomnia symptoms, while male sex (AOR: 0.88, 95% CI: 0.77–0.99), Islamic religion (AOR: 0.75, 95% CI: 0.66–0.86), has caring friends (AOR: 0.91, 95% CI: 0.85–0.98), and higher physical activity (AOR: 0.96, 95% CI: 0.94–0.98) were negatively associated with insomnia symptoms. Furthermore, there was a significant indirect effect of loneliness (explaining 29.2%) and multisite pain (explaining 30.3%) on insomnia via multimorbidity. Almost one in three aging adults reported insomnia symptoms. Loneliness and multisite pain were found to mediate the association between multi-morbidity and insomnia problems. Reducing loneliness and multisite pain may help in reducing insomnia symptoms in aging multi-morbid patients.

When dying worms look alike: Convergent pathology may create mechanistic illusion in plant-derived anthelmintic research

ABSTRACT The recent study by Devi et al. provided compelling ultrastructural and histological evidence of severe damage in Ascaridia galli following exposure to Meyna laxiflora seed extract, supporting its anthelmintic potential. However, the interpretation of such morphological alterations warrants further consideration. Similar cuticular and internal lesions have been reported in parasites exposed to chemically unrelated compounds with distinct molecular targets, suggesting that comparable structural phenotypes may represent convergent pathological endpoints rather than mechanistic signatures. This raises the possibility that morphology-based interpretations could inadvertently create a false mechanistic consensus in plant-derived anthelmintic research. Histopathology and scanning electron microscopy are powerful tools for demonstrating efficacy, but they may not reveal the initiating biological events underlying parasite death. Distinguishing convergent pathology from shared pharmacology is therefore essential for the rational development of botanical anthelmintics. Future investigations should focus on mechanism-oriented approaches capable of identifying upstream molecular and physiological disturbances preceding structural collapse. Understanding why dying worms look alike may prove as important as discovering new compounds capable of killing them.

Prospective feasibility study of a telehealth platform for community-based NCD risk assessment by community health leaders in rural northern Thailand

Introduction Limited access to preventive healthcare in rural and remote areas poses a persistent challenge for early identification of non-communicable disease (NCD) risk. We evaluated the feasibility and acceptability of HealthD , a telehealth-supported platform that enables Community Health Leaders (CHLs) to conduct community-based NCD risk assessment and facilitate follow-up in rural northern Thailand. Methods We conducted a prospective, single-arm feasibility and acceptability study from June to November 2023 in Chiang Mai Province, Thailand. A total of 120 adults aged 30-70 years without a prior recorded diagnosis of the target NCDs were enrolled. Trained CHLs used HealthD to perform community-based assessment for diabetes mellitus (DM), cardiovascular disease (CVD), and respiratory symptom/functional limitation screening using guideline-informed algorithms and field-based measures, followed by risk-tailored counseling or teleconsultation at two- and four-month follow-up. Feasibility was assessed through recruitment, retention, CHL competency, workflow delivery, and acceptability among CHLs and participants. Results Of 126 individuals approached, 120 were enrolled (recruitment rate 95.2%), and 119 completed follow-up (retention rate 99.2%). CHL competency improved after training, with mean assessment scores increasing from 8.4 to 9.3 ( p = 0.010). Acceptability was high among CHLs (92%) and participants (97%). The HealthD workflow, including in-person assessment, point-of-care testing, digital data entry, algorithm-based risk-output generation, and teleconsultation follow-up, was implemented in the study setting. Cytotoxic Effect of Halimeda macroloba Crude Extract on Cervical Cancer Spheroid Cells via p-mTOR/mTOR, Apoptosis, and Ferroptosis Pathways

Background: Because of the limited efficacy of existing therapeutic strategies and the emergence of treatment resistance, cervical cancer remains a major global health burden. Recently, marine-derived natural compounds have attracted attention as potential sources of novel anticancer agents. We aimed to investigate the cytotoxic effects of crude Halimeda macroloba extract on cervical cancer spheroid cells and to explore its underlying molecular mechanisms. Methods: Three-dimensional spheroid cultures of SiHa cervical cancer cells were treated with varying concentrations of H. macroloba extract, followed by assessment of migration, cell viability, caspase activity, and protein expression profiles using liquid chromatography–tandem mass spectrometry, with selected findings validated by Western blotting. Results: Treatment with H. macroloba extract significantly reduced spheroid migration and decreased cell viability in a dose-dependent manner, accompanied by increased caspase activity. Proteomic analysis demonstrated altered expression of proteins associated with mitochondrial function, apoptosis-related pathways, and oxidative stress-related processes. Mechanistic studies further revealed downregulation of phosphorylated mTOR (p-mTOR) and total mTOR, together with increased expression of pro-apoptotic markers. Conclusion: The H. macroloba crude extract exerts anticancer effects in cervical cancer spheroids, primarily through apoptosis-associated mechanisms and modulation of the mTOR signaling pathway, supporting its potential for further investigation as a marine-derived bioactive compound. Chikungunya virus infection: A comprehensive review of pathogenesis, diagnosis and management

Chikungunya fever (CHIKF), caused by the chikungunya virus (CHIKV), is an acute mosquito-borne viral disease that poses a rising global public health threat. To date, no specific antiviral therapies are available, and precise diagnosis is pivotal for optimal clinical management. This review systematically summarizes the latest evidence on CHIKV pathogenesis, diagnostic strategies, and clinical management based on English literature published between 2004 and 2025. Virus-host interactions dominate disease progression, including viral entry mediated by E1/E2 envelope proteins binding to host Mxra8 receptors and suppressed type I interferon responses via nsP2-mediated inhibition of the JAK-STAT pathway. The E1-A226V mutation greatly enhances viral transmissibility in Aedes albopictus, fueling the global spread of CHIKV. Clinically, persistent chronic arthropathy serves as the major long-term complication in 25%–40% of infected patients. Current diagnostic approaches consist of real-time RT-PCR as the early gold-standard test and serological antibody detection, whereas serological cross-reactivity with other arboviruses and inconsistent detection results frequently cause clinical misdiagnosis, and the confirmatory plaque reduction neutralization test (PRNT) is impractical for routine use. Clinical care for CHIKF remains predominantly supportive, relying on symptomatic treatment and close patient monitoring. Two FDA-approved vaccines have been licensed via accelerated approval based on immunogenicity surrogates, with solid clinical protective efficacy yet to be validated. Loneliness, rumination, and adolescent psychological crisis in China: a pilot moderated mediation study

Background Adolescent psychological crisis—encompassing depressive disorder, anxiety disorder, and suicidal ideation—is a major public health challenge. One in five adolescents experiences a clinically significant mental health condition globally. Loneliness is a well-documented, modifiable risk factor for these outcomes. However, the exact cognitive-emotional pathways linking loneliness to acute psychological crisis remain unclear. This is particularly true in China, where rapid social change and intense academic pressure increasingly strain adolescent mental health. We do not yet fully understand how protective resources might buffer these specific pathways. Objectives and methods This pilot study tests a moderated mediation model based on the Evolutionary Theory of Loneliness and the stress-vulnerability framework. We investigate ruminative thinking as a cognitive-emotional mediator between loneliness and psychological crisis. We also test whether perceived social support and psychological resilience moderate distinct stages of this pathway. Based on a pilot sample of N = 312 Chinese adolescents (ages 12–18), we use structural equation modelling (SEM), confirmatory factor analysis (CFA), and Hayes’ PROCESS Macro (Model 14) with 5, 000 bootstrap replications. Two-wave longitudinal data ( n = 187, 8-week interval) were available for the depression outcome only; the anxiety and suicidal-ideation pathways were examined cross-sectionally. Results Loneliness is significantly associated with depressive symptoms (β = 0.43, p < 0.001), anxiety (β = 0.38, p < 0.001), and suicidal ideation (β = 0.27, p < 0.001). Evidence for Step-Edge-Assisted Large Hole Borophene on Ni(111)

Due to its unique electronic properties, boron tends to form various multicenter bonds, enabling the construction of diverse two-dimensional borophene structures through modulation of the number and distribution of boron vacancies within a triangular lattice. In this study, we investigate a large-hole borophene model on heated Ni(111) substrates via molecular beam epitaxy under ultra-high-vacuum conditions. The complex atomic arrangement and electronic structures were investigated by scanning tunneling microscopy, low-energy electron diffraction, angle-resolved photoemission spectroscopy, and density functional theory calculations. Our results suggest that boron atoms are initially incorporated into the Ni surface and preferentially nucleated at step edges. The favorable geometric matching between the proposed structure and the Ni step-edge configuration contributes to the stabilization of the structure and facilitates its formation under the present growth conditions. These findings broaden the structural diversity of 2D borophene sheets and offer insights into the controlled synthesis of complex borophene architectures on metallic substrates.

Geospatial determinants of diabetes risk in Thailand: socioeconomic and health service factors

Diabetes prevalence is increasing in Thailand, creating growing demands on the health system. Understanding the spatial distribution of diabetes risk and its association with socioeconomic and healthcare system factors among the diabetes risk population is critical for designing targeted prevention and intervention strategies. We examined the distribution of diabetes risk groups across provinces in Thailand with reference to the spatial association between economic, social and public health service factors based on data from the Ministry of Public Health's Health Data Center (HDC) for the year 2021. The dataset included 22,491,934 individuals across the 76 provinces as well as social, economic and public health services. The methods included Local Indicators of Spatial Association (LISA), Ordinary Least Squares (OLS), Spatial Lag Model (SLM) and Spatial Error Model (SEM). Explanatory variables included average night-time light intensity, average monthly income, hospital-to-population ratio and proportion of the population with health insurance. Clear aligner therapy for early orthodontic treatment in children aged 6–13 years: A scoping review of clinical performance and evidence gaps

BACKGROUND: Clear aligner therapy has expanded substantially in mixed dentition, yet the boundaries of clinical predictability across indications, movement types, and growth stages remain incompletely defined. The aim of this review was to synthesize evidence on clear aligner therapy in mixed dentition, describe indication-specific performance patterns, and identify evidence gaps that limit clinical confidence. METHODS: This scoping review was reported in accordance with PRISMA-ScR. PubMed/MEDLINE, Embase, and Scopus were systematically searched for studies published from 2000 to 2025. Additional records were identified through Google Scholar and reference list searching. Randomized controlled trials, prospective and retrospective cohorts, and cross-sectional studies were eligible. Data were charted and synthesized descriptively. RESULTS: Fifty-nine studies were included (8 randomized controlled trials, 11 prospective cohorts, 38 retrospective cohorts, 2 cross-sectional), most involving children aged 7 to 12 years. Achieved transverse dentoalveolar expansion with clear aligners was lower than planned, particularly posteriorly and at gingival levels. Comparative studies suggested greater posterior and palatal changes with rapid maxillary expansion. Mandibular advancement aligners were associated with Class II sagittal improvement, with responses varying by skeletal maturity and dentoskeletal pattern. Evidence for incisor correction, molar movement, vertical outcomes, airway, oral health, patient-reported outcomes, and smile esthetics remained limited. The association between the American Heart Association Life’s Essential 8 and depression trajectories among postmenopausal women: findings from National Health and Nutrition Examination Survey 2011–2020

Depression is a psychological condition characterised by alterations in the mood and behaviour regulation in women experiencing 50% greater burden of depression than men (1) . Additionally, cardiovascular disease (CVD) is the leading cause of death in women globally while both CVD and depression can predict the incidence of each other reciprocally. Postmenopausal women particularly, during the menopause transition, are at increased risk of developing CVD and depression (2) . Life’s Essential 8 (LE8), is a recently developed cardiovascular health assessment index comprising eight health behaviours and factors: dietary quality, physical activity level, nicotine exposure, sleep health, body mass index, blood glucose, non-high-density lipoprotein cholesterol, and blood pressure (3) . It may be used to predict the risk of depression prevalence. This study investigated the association between LE8 and depression prevalence among postmenopausal women using the National Health and Nutrition Examination Survey (NHANES) 2011–2020 data. The LE8 score was calculated as the average of all eight components categorised into low (0–49), moderate (50–79), or high (80–100) cardiovascular health status (3) . Depression severity was assessed using the Patient Health Questionnaire-9 (PHQ-9) (4) . Complex survey-adjusted regression models were used. A total of 5472 postmenopausal women were included (mean age: 63.0 years), of whom 654 (12%) met the criteria for depression. The LE8 scores (mean ± SD) for those with and without depression were 53.2 ± 14.6 and 64.9 ± 14.7 (p < 0.001), respectively. Development of data-driven framework for the geotechnical behavior of xanthan gum-treated clay reinforced with polypropylene fibers

This study develops a robust data-driven modeling framework to predict the unconfined compressive strength (UCS) and stiffness (Go) of low-plasticity clay soils stabilized with Xanthan Gum (XG) and Polypropylene Fiber (PPF), aiming to advance sustainable geotechnical design. A total of 108 soil specimens were prepared with varying XG dosages and cured over different periods, and predictive models were constructed using a Decision Table algorithm optimized with six bio-inspired optimization techniques. Among these, the Firefly-optimized model consistently provided the highest accuracy, demonstrating reliable agreement between predicted and measured values. Sensitivity analysis identified XG dosage, curing time, and dry density as the most influential factors governing UCS and Go. These findings highlight the strong potential of the proposed machine learning framework to guide field engineers in optimizing mix design parameters for improved mechanical behavior of bio-treated soils, reducing reliance on time-consuming and costly laboratory tests while promoting environmentally sustainable foundation practices. The need for this study arises from the growing demand for green soil stabilization techniques that minimize the use of cement and lime while still ensuring reliable performance in construction. Its applicability extends to real-world geotechnical projects such as embankments, road subgrades, and shallow foundations, where predictive modeling can significantly streamline design decisions and improve long-term sustainability.
