INTRODUCTION: Chronic gastroduodenal disorders remain challenging to manage, and new diagnostic approaches are needed to better delineate underlying causes and guide therapeutic decisions. Body Surface Gastric Mapping (BSGM) technologies combine high-resolution gastric myoelectrical activity measurements with symptom and psychological profiling to provide mechanistic insights into gastric motor and sensory dysfunction. An International Working Group convened to derive the first consensus classification of BSGM phenotypes (the "Auckland Classification"). METHODS: A Technical Group conducted a systematic literature and clinical database review to identify objective test biomarkers and candidate disease mechanisms. Evidence was synthesized across 50 studies (primarily in gastroparesis, chronic nausea and vomiting, and functional dyspepsia), and BSGM phenotypes were mapped to existing treatment guidelines. Subsequently, iterative review and development of consensus was performed by a Consensus Group composed of international clinical experts familiar with BSGM. Eleven statements underlying the classification were then derived and circulated as a final survey to establish agreement. RESULTS: Six BSGM phenotypes were endorsed: three defined by myoelectrical abnormalities (Dysrhythmic, High Frequency, and Low Meal Response) and three by characteristic symptom associations (Sensorimotor, Continuous, and Delayed Onset Symptoms). Published studies plausibly linked these phenotypes to mechanisms including interstitial cell of Cajal depletion, vagal impairment, hypomotility, visceral hypersensitivity, gut-brain dysregulation, and small bowel dysfunction. OBJECTIVE: This study developed and validated monolingual and bilingual sentence-bidirectional encoder representations from transformers (SBERT) models for detecting cancer recurrence within Thai-English electronic medical records (EMRs) from Thai cancer hospitals. METHOD: A multicentre dataset of 32 436 documents from 1250 patients was used for model development. External validation involved an independent dataset of 9244 documents from 384 patients across two Thai cancer hospitals. Performance was benchmarked against a fine-tuned PubMedBERT (MetBERT). RESULTS: The development dataset included breast (43.9%), colorectal (12.1%), cervical (28.0%) and head and neck (16.0%) cancers. MetBERT achieved the highest area under the precision-recall curve (AUPRC) for locoregional versus no recurrence (11.1%) and locoregional versus distant recurrence (91.7%), while monolingual-SBERT excelled at distant versus no recurrence (32.0%). External validation demonstrated MetBERT superiority for locoregional versus no recurrence (9.30%-21.50%). For distant versus no recurrence, bilingual-SBERT performed best with AUPRC 17.55%-24.39%. While MetBERT led in distinguishing locoregional versus distant recurrence (88.30%-94.70%), bilingual-SBERT demonstrated robust external validation performance (AUPRC 85.25%-91.80%). DISCUSSION: Low AUPRC values (9%-32%) reflect the extreme class imbalance in real-world data (~1% recurrence prevalence). Despite this, fine-tuned MetBERT achieved highest performance, while bilingual-SBERT demonstrated superior robustness during external validation. The European Society for Medical Oncology (ESMO) Clinical Practice Guidelines for the diagnosis, treatment, and follow-up of patients with hepatocellular carcinoma (HCC) were published in May 2025 and were adapted in August 2025, according to previously established standard methodology, to produce the Pan-Asian adapted ESMO consensus guidelines for the management of Asian patients with HCC. The adapted guidelines presented in this manuscript are the consensus opinions reached by a panel of Asian experts in the treatment of patients with HCC representing the oncological societies of China (Chinese Society of Clinical Oncology), Indonesia (Indonesian Society of Hematology and Medical Oncology), India (Indian Society of Medical and Paediatric Oncology), Japan (Japanese Society of Medical Oncology), Korea (Korean Society of Medical Oncology), Malaysia [Malaysian Oncological Society (MOS)], the Philippines (Philippine Society of Medical Oncology), Singapore (Singapore Society of Oncology), Taiwan (Taiwan Oncology Society), and Thailand (Thai Society of Clinical Oncology) and coordinated by ESMO and the MOS. The voting was based on scientific evidence and was independent of current treatment practices, drug access restrictions, and reimbursement decisions in the different regions of Asia. The latter are discussed separately in the manuscript. The aim is to provide guidance for the optimisation and harmonisation of the management of patients with HCC across the different countries of Asia, drawing on the evidence provided by both Western and Asian trials. Attention is drawn to the disparity in the drug approvals and reimbursement strategies between the different regions.
Improving feed efficiency while maintaining gut stability is a key nutritional objective during the post-weaning period in piglets. This study investigated the effects of dietary supplementation with a microbial bioactive complex (MBC), composed of Pediococcus pentosaceus CACC616, Lactobacillus reuteri CACC607, L. dextrinicus CACC889, L. pentosus CACC891, and Saccharomyces cerevisiae CACC699, on feed efficiency and gut microbial fermentation in weaned piglets. The complex was produced via solid-state fermentation (SSF), enabling the co-delivery of viable microorganisms and fermentation-derived metabolites. A total of 34 piglets were randomly assigned to either a basal diet (control) or the same diet supplemented with MBC for 33 days. The MBC contained approximately 2.4 × 10 9 CFU/g of lactic acid bacteria and 5.17 × 10 6 CFU/g of yeast and was included in the diet at 0.5%. MBC supplementation significantly improved feed conversion ratio ( p < 0.05) and reduced average daily feed intake, indicating enhanced feed utilization efficiency without adverse effects on health-related blood parameters. The constituent strains exhibited desirable probiotic and safety characteristics, including tolerance to acidic and bile salt conditions, adhesion to porcine intestinal epithelial cells (IPEC-J2), absence of hemolytic activity, and low biogenic amine production. Microbial analysis revealed that taxa enriched in the MBC-treated group, including Succinivibrio and Succinivibrionaceae UCG-001 , are associated with carbohydrate fermentation and succinate-producing pathways. Underage alcohol use and bullying are major adolescent health concerns in the Philippines. We estimated the prevalence of current alcohol use and its association with bullying victimization. We analyzed data from the 2019 Philippine Global School-Based Student Health Survey (GSHS). Current alcohol use was ≥1 drinking day in the past 30 days. Bullying exposures included school-based, outside-school, and cyberbullying in the past 12 months. Weighted chi-square tests and multivariable logistic regression were used to identify predictors. The current alcohol use prevalence was 20.4%. Alcohol use was more likely among males (aOR = 1.77; 95% CI: 1.58–1.98). Adolescents < 15 years had lower odds than those ≥15 years (aOR = 0.55; 95% CI: 0.49–0.61). Bullying victimization was independently associated with drinking, strongest for cyberbullying (aOR = 2.36; 95% CI: 2.08–2.68), with additional associations for school-based (aOR = 1.39) and outside-school bullying (aOR = 1.51). Underage drinking is closely linked to bullying, particularly cyberbullying, supporting integrated school-based prevention combining psychosocial support, digital literacy, and gender-responsive strategies.
Context Although once widespread, tuangi (cockle, Austrovenus stutchburyi), a taonga (sacred) species for local Māori people, have undergone significant declines in Ōhiwa Harbour, prompting local-led aspirations for their restoration. Aims This study demonstrates how balancing mātauranga Māori with ecological modelling can improve understanding of species and habitat relationships and support estuarine management. Methods We combine contextual historical mapping with a predictive Species Distribution Model (SDM) to identify suitable habitats for assessing and monitoring areas for a co-managed and co-implemented shellfish surveying and monitoring program in Ōhiwa Harbour, Aotearoa New Zealand. Species distribution and relative abundance were modelled using a SDM parameterised with Indigenous Knowledge and empirically derived environmental data informed by Iwi (tribal)-identified traditional harvesting areas.
